Halfway House

A community- based residential treatment and rehabilitation facility that provides services for chemically dependent persons in a supportive, chemical-free environment.

Holistic and Wellness Approach

Services are designed to enhance the development of the whole person; care transcends a narrow focus on symptom reduction and promotes wellness as a key component of all treatment and support services. Services are strengths based, culturally competent and trauma-informed.

Home and Community Based Services

Services and supports provided in a home or community location to help an individual as independently as possible.

Home and Community Services Information System (HCSIS)

A Web-enabled information system that serves as the information system for DPW program offices that support the Home and Community-Based Services Waivers (including ODP).

Homeless Outreach/Café

Staff who reach out to persons experiencing street homelessness, offering food and clothing and access to emergency shelter, safe havens, and entry-level residential treatment; staff who engage visitors to homeless cafés and assist in accessing behavioral health treatment and services; links to permanent supportive housing.

Hope
